Postdischarge Telehealth Support for Caregivers Through a Coaching Approach: A Case Study

Abstract Date Presented 04/02/2022 Coaching care partners through the transition from hospital to home can be beneficial for both caregiver and recipient. In this case study, we describe benefits of coaching postdischarge a telehealth coaching-in-context approach. Both made remarkable gains in occupational performance, self-efficacy, quality life. The preliminary data presented here are part pilot randomized controlled trial that compares with usual care.
